**  PLEASE READ CAREFULLY as this form has changed!  **

The directory will be updated on or around the 1st and 15th of each month. What you submit will be added to the   
directory how it is received, so please make sure your information and spelling is correct.

The "required" element on many of the sections has been removed so that updating your information is faster.   
However, if you are submitting a NEW listing, please make sure to fill out all of the sections so your entry is    
complete.    

If you are UPDATING your information, fill in your name and city (still required) and then
fill in only the information  
that has changed.  Skip anything that remains the same
.  If you need to view your listing, click on Parents tab
above, choose Finding Childcare, then select your city.

You must have a current membership to be included in the Provider Directory.  Your membership will be verified before  
adding your listing. If you need to renew your membership, the quickest process is using our online link using the
Providers tab above. Click on Membership on left sidebar of Providers Page.  There is also a mail-in option.
